INTRODUCTION :
Connell E.(2008) .Listening is a part of our daily life activity .In order to build a good and effective communication process, effective listening and reflective responses to the content and the feelings are to be exhibited. Cacchione.P,(2011)., Active listening is a skill in communication and when it comes to therapeutic communication this plays the vital role. This skill must be developed. Mrs. Johns, when admitted, she is provided nursing care effectively by ruling out all the sensory deprivation problems. Elizabeth. C,(2011).,Visual defects, hearing defects and anxiousness because of the diabetic foot ulcer should be mainly focused and nursing care should be provided accordingly. Special therapeutic communication techniques should be incorporated during the nurse-client interpersonal relationship.
1.THERAPEUTIC LISTENING RESPONSES WHEN CARING FOR MRS.JOHNS
Elizabeth.C,(2011).,Therapeutic communication is a process which involves face to face interaction and focuses on enhancing the physical, physiological, emotional, psychological and social status of the individual.
Connell E.(2008).,Therapeutic listening responses are the reflective responses towards the communication process that is in proceedings. Effective listening is the basis for communication and it is the very difficult skill that every one fail to learn and practice. Listening is not as like a biological functioning, it is more than a biological functioning between the eyes and the brain.
Therapeutic reflective responses and its definitions are as follows. They are
1.Encouraging
2.Paraphrasing
3.Reflecting and
4.Summarising
Encouraging is a communication technique which acknowledge the individuals thoughts and feelings and making the person to converse more. This technique develops the communication. Rothwell, J. Dan (2010).
Paraphrasing is a communication technique and it is a response. This response clarifies the understanding about the content of a message and increases the objectivity of the communicator. Cortes, T., (2008).
Reflecting is the technique in which the listener provides an sensible thought that he is listening to the speaker and is understanding the message that is conveyed. Rothwell, J. Dan (2010).
Summarising is essential to proceed further in the conversation. It comprises of the total ideas, facts and the contents that are involved in the message. Cortes, T., (2008).,
The responses that could be used when caring for Mrs. Johns are as follows :
When caring for Mrs. Johns, the staff nurse can encourage her to ventilate her anxiousness and feelings in order to make her feel comfortable and provide a positive approach. This would help Mrs. Johns to develop a trust worthy relationship with the nurse. Cacchione,P.(2011)., The nurse can encourage her by making her to speak out her past experiences or the reason for her worries and anxiousness. She can acknowledge it by asking, what happened to u ? with a exclamatory tone "its really interesting", I see, great etc.
Using the technique paraphrasing: of active listening is while Mrs .Johns is letting out her thoughts or the distressing experiences the nurse should apply the technique of paraphrasing. She can Make Mrs. Johns let out by asking few questions about her health and thereby understanding the message content and acknowledging.
The nurse should use this technique in order to promote a sense to Mrs. Johns that she is listening to her.
In order to save time by conveying to the client indirectly that she has already discussed about this. by saying, "In other words is this is your condition?", "You are experiencing pain very badly?" "you hate staying immobile?".etc. Fassaert T .,(2007).
When the nurse feels the Mrs. Johns develops a thought that the staff nurse is not listening to her.
When Mrs. Johns is repeating the same messages again and again.
If this response is not used properly it would lead to unnecessary wate of energy by the client. It increases the fatigue level. The working time of the nurse is affected by listening to same old feelings etc.
Reflecting technique acknowledges the emotion, message that is communicated verbally and non-verbally. This technique should be used in order to make the speaker understand that what Mrs. Johns is conveying is understood by the nurse. It provides an assurance that the staff nurse is listening to the client. Fassaert T .,(2007)., It increases the understanding of the staff nurse, it clarifies their thoughts and it reassures that the staff nurse is ready to help her and listen to her.
The staff nurse can use this technique when the client gives a pause and expects for the reply from the staff nurse
When Mrs. Johns tells the nurse that she is better after her distressing experience and the symptoms that she experienced, the nurse can use this technique. And can state as "You look relieved," ,"You appear confused." You seem to feel anxious because you are immobilized.
Possible outcome is that, the client would develop a negative thought, feel isolated, experience low self esteem and develop thoughts that the nurse is not listening to her, the nurse is not interested to converse with her etc.
The nurse can summarise the thoughts and the feelings that Mrs. Johns has conveyed to her by acknowledging the messages that she understood from the conversation. This allows for building up the conversation. Fassaert T .,(2007)., She can summarise by saying ,"It seems to be the signs and symptoms that you exhibit and experience".
When Mrs. Johns is getting overwhelmed, or when she experiences fatigue, etc. The nurse can use this technique when Mrs. Johns is getting exhausted or when she is repeating the same topics repeatedly. If the staff nurse fails to use this technique then the client would experience some fatigue, get over exhausted, may develop a sense of thought that the nurse has not listened to her, it is a waste of time for the nurse where she can work at that time. The nurse can use it when the client begins to say her day of admission onwards the treatment that is provided, pain, her life history etc.
These are the various active listening strategies or reflective responses in therapeutic communication. Listening is essential and responding to the listener is more important as well.
