Case Progress: Johnny Bravo

Permanency Goal: Reunification

Progress/Assessment of Child:

Since Johnny's placement at Youthlinks he has adjusted quite well. Johnny is attending school on a regular basis and completes his homework in a timely manner. Johnny has been role model to the other youth in the facility. Johnny is always the first to volunteer. He participates and interacts well with others. Johnny reports he would rather be at home with his family. Johnny has stated he now understands how important it is to go school on a daily basis and turn in all of his homework. At this time, staff at Youthlinks does not have any concerns regarding Johnny.

Johnny's mother, Polly, is very involved. Polly transports Johnny to and from his appointment. Polly always brings Johnny back on time. The staff at Youthlinks observed that Johnny and his mother have a great relationship. Polly calls to check on Johnny at least twice a week if not more.

Physical Health: Johnny is currently in good physical health. He had his physical on May 15, 2012 at Alegent Health Clinic on 84th Center. He was seen by his primary care physician Dr. Squarepants.

Johnny was seen by Dr. Star at Pearle Vision. Johnny was given a new prescription for his glasses.

Johnny has an appointment scheduled on June 19th to see the dentist.

Mental Health: Johnny currently sees Wilma Flintstone weekly at Capstone.

Developmental: Johnny is developmentally on target for his age. Johnny has been diagnosed with

ADHD.

Medications: Johnny is currently taking 10mg of Focalin XR twice a day.

Education: Johnny is in the 10th at Central High School. Since Johnny has come into care he has not missed any days of school. Johnny does have an IEP to address his ADHD needs. Johnny has brought his grades up to passing.

Visitation: Johnny is currently having unsupervised visits with his mother. Due to Johnny's behavior he has been granted weekend and overnight passes. Johnny always returns on time from his visits. At this time the staff does not have any concerns regarding visitations.

Recommendations: The staff at Youthlinks recommends that Johnny be returned to his mother care. Johnny is doing well. Johnny gets along well with others and respects all staff members.
