Dear Ms. Cecelia,

                                Notice of Termination - Lily Cecelia

We regret to inform you that your employment as Assistant Accountant with ACK Sdn.Bhd. is hereby terminated. Your last day serving the company will be on 14th October 2015. We have decided to terminate your employment for the following reasons:

1. On 12 March 2015, you were given a warning letter for job performance because each of your last three bi-weekly fiscal reports had been late and contained numerous errors.

2. On 5 April 2015, you were given a second warning letter because you failed to submit a written proposal for the grant fund tracking system in a timely manner.

3. On 11 June 2015, you were given a third warning latter because you were using rude language to communicate with our supplier GD concrete. This letter also indicated that your employment may be terminated if your job performance did not improve by 28 August 2015.

        We consider that your job performance is still unsatisfactory and have decided to terminate your employment because your fiscal report had been late submitted for 4 times after the first given warning letter.

Based on your length of service, your notice period is 3 weeks. Therefore your employment will end on 14th October 2015. In lieu of receiving that notice, you will be paid the sum of RM 2000.

You will also be paid your accrued entitlements and outstanding remuneration, including superannuation, up to and including your last day of employment. Thank You.
