Instructions: The following is a list of statements dealing with your general feelings about yourself. Check off the box of the response that best describes your response to the item.

3. I can do things as well as anyone else.

8. I do not live up to others' expectations.

9. I believe I have much to be proud of.

10. I often think negatively about myself.

11. I do not feel that I have enough friends on social networking sites.

12. People have told me that I have a negative attitude.

13. I treat my body well, by exercising and eating right.

14. My life does not have meaning.

15. I am important to my family and friends.

16. I criticize my physical appearance often.

17. Social networking sites prove to me that I am not as important as other people.

18. People always are better than me at things.

19. I possess more good qualities than bad.

20. I often feel successful.

To score the items, assign a value to each of the 20 items as follows:

o For items 1,3,4,5,7,9,13,15,19 & 20: Strongly Agree=4, Agree=3, Neutral/No opinion=2, Disagree=1, and Strongly Disagree=0.

o For items 2,6,8,10,11,12,14,16,17, & 18 (which are reversed scored): Strongly Agree=0, Agree=1, Neutral/No opinion= 2, Disagree=3, and Strongly Disagree=4.

The scale ranges from 0-80. If a person scores from 0-15, they have extremely low self-esteem. If the person scores from 16-31, they have lower than average self-esteem. A score of 32-47 is considered average self-esteem. 48-63 is above average self-esteem and 64-80 is extremely high self-esteem.
