The same system will be used for continuous quality improvement. There will be weekly analyses of "successes" and "failures" related to the project. For example, if a particular region reports a new strategy for preventing admission or a new diagnosis that should be targeted then the success will be quickly shared with all other regions to assess and deploy the successful results where appropriate. Similarly, there will be weekly reporting on failures in strategy or execution. For example, if there is an opportunity to prevent a hospital admission and the patient was still admitted then there will be a case review to determine what adjustments can be made to achieve success in the future.

Both successes and failures will be reported to the Chief Medical Officer to assess what measures can be taken to improve the quality of care and safety of patients as they are seen by providers and partners who are participating in the project. The same reporting will be shared with participating health plans and the respective Chief Medical Officers of the plans will be encouraged to review the reporting and results of the project.

Both patients and families will be surveyed on their experiences. Particular attention will be paid to the experience of telemedicine in new environments such as Assisted Living Facilities. The results of the surveys will be reported to all members participating in the project including monitoring contractors and health plans.
